Regaining Independence

Integrated Discharge Team (IDT)

The Isle of Wight NHS Trust has an exciting opportunity for a Registered Nurse, HCPC Registered Therapist or Social Worker, to join the Integrated Discharge Team (IDT). We are looking for an individual who will be a key member within the IDT, committed to providing the highest standards of support and care.

Currently based in the Integrated Discharge Team you will be responsible for assessing patients for what they need for a safe and appropriate discharge from hospital. The post holder will work within the acute hospital setting and rehabilitation to identify through assessment, appropriate rehab candidates and non-rehab candidates to follow the correct pathway for discharge from hospital. This will follow National guidance.

The post holder will be NMC or HCPC registered and have relevant previous UK NHS experience. The post requires comprehensive experience developing and supervising more junior members of the team.

IDT are within the Regaining Independence cluster. The team includes Nurses, Therapists, Social Workers, Reablement, Age UK, Red Cross, Carers Lounge, Patient Pathway Navigation Team, Site Management Team, Single Point of Commissioning. We work closely with our Nursing home Residential homes and care providers and GP’s.
