Community Division – Specialist Services

Associate Community Matron with interests in Neurology/Parkinson’s Disease/ Long-Term Conditions

Band 6

Full time – 37.5 hours per week – Contract Permanent

An exciting opportunity has arisen for a committed and enthusiastic and experienced individual to join our forward-thinking Community Division Team in a new post as an Associate Community Matron (with interests in Neurology/ Parkinson’s/ Long-Term Conditions, working across Community, Primary and Secondary Health care settings)

The Isle of Wight NHS Trust is an advancing Integrated Trust providing Acute, Mental Health, Ambulance and Community Services.

You would be joining our Community Division at an exciting time as we progress on our journey to outstanding, having achieved a CQC rating of good overall in 2019 including Outstanding for Care in 2021. Our transformational achievements include the extensive use of technology enabled care and the development of integrated multi-disciplinary and multi-agency teams, in conjunction with a wide range of partners across the Island and mainland.

This new opportunity will be pivotal to the development and implementation of the next step in integrated working in a multi-professional arena. Additionally, the post of Associated Community Matron will be supported through the network of community Matrons who are well established in managing long-term conditions.
