Pembrokeshire Community Nursing Services are looking for dynamic and enthusiastic Band 5 nurses to join our multi-disciplinary team at South Pembrokeshire Community Hospital.

Sunderland Ward is a 40 bedded ward that has a strong ethos of rehabilitation and recovery alongside palliative and end of life care. The Ward utilises a co-ordinated, joined up, integrated model of working, putting the patient at the centre of the care that is provided. The integrated team within the ward at SPH work closely to support the health and well-being of patients, nursing them closer to home and setting goals for individual optimum independence and dignified, compassionate care provision. Community nursing care is often complex and challenging and requires nursing staff that are enthusiastic, empathetic, organised, critical thinking, reflective and knowledgeable.

Main duties of the job

As a nurse led ward, you will be able to use your knowledge and experience to work autonomously in a variety of clinical areas including cognitive impairment/dementia, frailty, palliative and end of life care needs, complex wound care management and rehabilitation, working closely and interactively with multi-disciplinary teams.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales. We do this in partnership with our three local authorities and public, private and third sector colleagues, including our volunteers, through:

Four main hospitals: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;

Seven community hospitals: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;
