Overview
The Clinical Scheduler is responsible for performing numerous clerical activities related to scheduling primary medical and specialty appointments plus nursing clinic visits for resident/participants. Schedules/ reschedules appointments in order to maintain the highest quality of coordinated care for the resident/participants. May assist with other clerical duties in absence of other clerical staff.
Responsibilities
Job duties specific to this position:
- Schedules all medical and medically related services for resident/participants or participants; schedules all primary appointments per provider requested timeframe; schedules all contracted specialty appointments after provider approval utilizing the scheduling system; completes pre-authorization process; also schedules laboratory and radiology and other testing appointments with outside vendor. Completes reconciliation process. Notes if appointment was not attended/cancelled documenting who cancelled and reason why if known.
- Facilitates/completes all communications related to scheduling of all appointments which includes appointment cards, notification calls to caregiver/family as requested; verbal and written communication to contracted skilled nursing and enriched living facilities as necessary; notification to home care scheduling department for aide escort request when needed, etc. When a resident/participant expires, reviews specialty appointments, calls specialist offices to cancel appointments and then follow up with formal notification letter.
- Supports front desk/clinic during busy hours of operation by answering the telephone and assisting resident/participant and families as appropriate or by taking a message. May also perform duties of receptionist in his/her absence, including but not limited to filing additional paperwork.
- Arranges for last minute transportation to/from appointments for resident/participants by notification of case manager completing necessary transportation PCP/MD paperwork.
- Actively participates in morning report, staff meetings or other meetings as requested by the Clinical Care Manager/Clinic Manager; assists and participates in relevant committees, task forces and special projects to promote quality improvement as requested.
- Practices safety awareness, reports any possible safety/environment issues to supervisor, and adheres to safety policies and procedures.
- Demonstrates adherence to all compliance policies and procedures and the code of conduct. Displays compliance oriented behavior in the workplace. Is responsible for promoting and fostering compliance in the workplace. Adheres to the mission and philosophy of the Loretto Corporation.
- Models and promotes service excellence by developing and maintaining positive respectful relationships with all customers, internal and external, to include resident/participants, families, team members, all staff throughout the organization and community partners; ensures the highest quality of care by performing responsibilities according to the highest professional standards.
- Demonstrates knowledge of Emergency Preparedness Plan and can verbalize their role during an event.
- Performs other duties as assigned by the supervisor.
PACE CNY Specific Job Duties
- PACE CNY uses the TruChart scheduling system.
- Prints pertinent participant information from TruChart as appropriate when preparing a request for specialty service(s) for an out of network provider; ensures appropriate signatures are obtained prior to making the appointment.
- Arranges for transportation to/from appointments for participants by notification of PACE CNY transportation coordinator and completing necessary transportation paperwork; assists Day Center Manager with setting up escorts as necessary; communicates daily or as needed with Transportation Director/Designee to review outside appointment schedule to confirm accuracy and to identify any potential conflicts, etc.

- Demonstrates knowledge of PACE/D&TC Emergency Preparedness Plan and can verbalize their role during an event.

Qualifications
- AA degree in health related field plus one year experience in medical office setting scheduling patients.
- Excellent communication and customer service skills required.
- Demonstrated organization, time management, and multi-tasking skills necessary; detailed oriented.
- Proficient with MS Office tools (Outlook, Word, Excel etc.) and able to learn new software rapidly.
- Accurate data entry skills required.
